This is a really unique take on the old happy/sad clown cliche. The smile looks like it has been drawn in crayon, or possibly makeup, which really makes it stand out against the lifelike seriousness of the face. It seems the message is that real life is full of suffering while happiness is a fake veneer that we show the world.
